- We need to learn more about you - in order to determine how we might best serve you, we will want to know about your financial circumstances, goals, objectives, problems, concerns and what you want out of an advisory relationship. You have certain responsibilities if our relationship is to be a success. For starters, we ask that you take 15 minutes to complete three brief confidential questionnaires in advance of our first meeting and return these questionnaires (via mail or fax) to our office listed below. The questionnaires are:

- The Mohr Group Advantage Client Data Sheet provides statistical and contact information about you and your family.
- The Mohr Group Advantage Client Profile helps us determine what your general planning and investment needs may be.
